Question about Process Control

With the diagram listed in the url above if steam in this system needs to be Fail-safe in case there is a problem?

Should the steam valve Fail-open or Fail-close? Should the PIC be direct-acting or reverse acting? Should the TIC be direct or reverse acting? Is the control loop considered a Feedforward or Cascade control loop?

Lets say that your valve fails open. What happens to the steam flow?

Can the steam source supply it?

Can the heat exchanger take the pressure?

Such questions are not simply a matter of control function, but issues of mechanical design.
