The Cause,
Due to the shield of the flexible pipe was disconnected from the original position and the pipe length extended(40cm). The extension cause stress on the pipe thus cracks the pipe and oil leaking under high pressure.
The question here is this approve and the cause of the fire. please let me know if this case happened some elsewhere in the world for GE Steam Turbines? and what is the advise for us due we have got other two units?
