Fault When Using Two S7 Redundant CPUs

Hello,

I am using S7-400H redundant CPUs for PCS7 system. While warm restarting CPUs from SIMATIC Manager, the secondary CPU goes in the couple mode very often. What can be the possible fault? I checked by replacing the fiber cable for redundancy.
