Solar Inverter Error Islanding Passive

We have the following solar inverter, which is tripping every few seconds, with error E1-5 (Islanding passive). What could be the real cause? Anyone experienced and solved this issue?

We checked the manual, and did not find any problem with the below points!

Inverter KP100L-OD-EU, solar grid-tie inverter 10 kW , Omron

Error E1-5: Islanding passive
Situation: Islanding operation detected by passive method due to error in the power from the grid.

Possible Cause:
1. AC connection is not correct.
2. Grid condition is weak or unstable.
3. Other high-power consumption device is affecting the grid system.
4. Inverter is abnormal.

Remedy for User:
1. If this problem occurs seldom (such as 1 time a day), no action is necessary.
Operation will restart automatically when the grid voltage returns to normal.
If the problem occurs frequently, do the actions below.
2. Find the device with high power consumption near your AC system.
3. Consult your utility power supplier, understand the grid conditions.
4. Ask help of your installer.

Remedy for Professional:
1. Check the system connection including polarities and security first.
2. Find the devices with high power consumption near the AC system.
3. Consult the utility power supplier, understand the grid conditions.
4. Use Inverter software to monitor the frequency and voltage.
5. If the situation is not improved, please replace the Inverter.

When did this problem start? Immediately upon initial power-up/commissioning? Or, just recently?

If the problem started just recently, what is the newest load (motor, computer, refrigerator, microwave, etc.) to be added to the system?

Was there a lightning strike/electrical storm recently?

What is the voltage at the output of the inverter before the inverter trips with this fault?

What is the frequency at the output of the inverter before the inverter trips with this fault?

Are you certain you have closely followed all of the manufacturer's installation instructions, including earthing?

Have you ensured the configuration of the inverter matches the application? For example, is the inverter set for Islanded operation if it's being operated independently of any other power system? Or, if it's connected to a larger power system is the configuration set for this mode of operation?

Is this inverter connected to a larger AC power system? Or, is it operating independently of any other power generator (wind turbine-generator, steam turbine-generator, engine-driven generator, hydro turbine-generator)?

If the inverter is connected to a larger AC power system, is the power system frequency stable? What is the power system voltage at the point the inverter is connected to the system?

Does the PV system power a storage battery, and the inverter convert the DC to AC?

You haven't provided enough information about the system for us to be able to provide much of an answer.
