Plant Status before Incident:

1. GT-1&2 were in Service with total 12.5 MW load. GT-1 @ each at 6MW each. GT @ 6.5MW.

2. GT-1 on liquid fuel and fuel transfer from liquid fuel to gas fuel was initiated as part of the GT-1 reliability test.

Plant status after incident:

1. GT1 load reduced to 6.0MW & fuel changeover initiated to gas fuel. GT1 tripped at 16:48 with following alarms:

"PT output bearing vibration high"
"GG axial displacement high"
"GT trip adjacent thermocouples failed deviating".

2. GT-2 load shoot up from 6MW to 12.2MW as load shedding not acted and GT tripped on

"GT trip turbine limiting temperature high."

3. Black out at Power plant due to both GTs tripping, EDG took auto start.

4. GT MCC loads were manually normalized to prevent EDG tripping.

5. GT-2 was started, but stopped at 17:47 hrs.as lub oil piping was vibrating heavily.

6. GT1 started & failed on GT start abort can flame failure tip temperature failed/deviating.

7. CPF-2 East substation power supply extended from RPP as Power plant GTs start up deferred.

Clearly you have a changeover and maybe maintenance problem.
Firstly the load shedding needs to be sorted to prevent future consequential failures.

Lubrication Oil pipe vibration could be caused by numerous reasons and needs to be checked out locally (point 5).

Liquid to Gas Changeover and the symptoms described suggest unburnt liquid in one or two of the 'cans'. Removal of igniters in the cans where the temperature dropped may confirm this.

Have you no maintenance contract? I understand the SGT400 can be remotely monitored under a contract called STA-RMS.

Thanks for your response.

Unit is under commissioning. but reliability test has been said successful as per Siemens personal.

No one has the clue of load shedding because no information or supporting documents provided by OEM regarding PMS system.

Liquid to gas changeover - Siemens didn't opened the CAN but they claim now the issue is solved. no clear reasons.

Regarding lube oil vibration - Siemens claims there was a air pockets and this causes the vibration. no root cause analysis report submitted on this issue.

regarding maintenance contract- we are behind Siemens all the time for LTSA. We have RDS contract with Siemens but after signing PAC only we can proceed further.
