Compact Logix Digital Output Module failure

Dear Sir,

As we have Compact Logix 1769 OB32 Digital Output Module (Transistor Outout), it was failed with unknown reason.

Could you please advise me what would be the problem and check list please?

Dear Satyam,
Kindly drive the outputs through interposing relay.Free wheeling diode should also be added to safeguard the output transistor of the card.
Also kindly mention complete configuration with Processor
