Flow Divider Failure

We have 6 bhel make gas turbine Mark-V operated, and 03 GTs are NP make of Frame-6. Out of 9 GTs, 4 are running with mixdc fuel (Liquid + Gas), liquid are LCO and Gas is Refinery fuel gas. Main problem is that in the last 2-3 months we have faced failure of Flow divider. We are running @25 MW load with 40% liquid and 60% gas. We are using additive SR-2008 winter grade for increasing lubricity of fuel.
 
How long have you been using the liquid fuel you're currently using? (By the way, what does LCO mean?)

How long have you been using the lubricity additive you noted?

Are all the flow dividers made by the same manufacturer?

Have you contacted the flow divider manufacturer or their local sales rep about the fuel you are using and the lubricity additive and the suitability of the flow divider for these two fluids passing through the flow divider?

Did the units operate on liquid fuel previously? The same liquid fuel as you're currently using?
