High Failure Rate for ICS Triplex system

we have in our plant an ICS Triplex Trusted system installed since 2001, but the problem is the high failure rate of input/output modules is increased dramatically. does any one have an idea about this system and how can we overcome and investigate the main cause of this high failure?
