How to configure the Tag address for special module A616AD

Hi all,
I use the Melsec OPC server to connect a Mitsubishi PLC A3A CPU-AJ71E71 ethernet card. I have a AD Converter Card A616AD, so I want to get analog data of 16 channels from the A616AD by OPC server, but I dont know how to input the Tag address for this device to get data in the OPC server. Before use OPC server to get analog values, I can get these values by command following:

Could you please help me convert this command to the Tag address or teach me how to configure the Tag address for the A616AD device? so that I can get the analog value by TOP server.
