I need a converter which should be used as an isolation barrier as well as a converter from 0 to -5V to 4-20mA signal.

 

Quite a strange requirement. Still any 0-5 v convert to 0-20 ma with isolation should be able to do this. You should reprogram either the source to 1-5 v or the final instrument to re scale to 4-20 ma.
