Modbus TCP Connection to Remote Virtual Server
Maybe you should config IP Address in VMWare to have same loop as PLC. Then change VMWare network setting to use bridge mode.

Maybe you should use SIEMENS IOT2040 that can communicate with S7 PLC or Modbus TCP connection and can be use with Node-Red that can communicate with MQTT for IoT device.

They are up to manufacturer design. If they need it they can use holding registers for read/write discrete or analog value. I have seen some devices use 4xxxx address for read only words, but for discrete I have never seen before. But I think if they use 4xxxx for boolean means if data is Off...

I think that modbus address that you monitor are 32 bit floating point but by default Modscan monitor in 16 bit Integer. You should go to Setup -> Display Options -> Floating Pt to display address in 32 bit floating point.

Today there are many android apps that can create mobile SCADA, but almost use Modbus TCP. If you can create mobile app that can communicate with multi protocol (Ethernet IP, Profinet), it is advantage for you.

Is the Module Index Equivalent to Modbus Slave Id?
Maybe I describe unclearly for your question. "Is module index 3 an industry standard or its vendor specific?" It is vendor specific. (Most devices use slave ID 1 but some use other ID).

Iconics Modbus Ethernet OPC Server
First use command prompt "ping Gateway IP Address -t" to check communication that good or not. Then try to use Modbus Scanner Software (ModScan, Modbus Poll) to check data that transfer from Modbus Gateway to device. If it is not stable, you should adjust timeout or reduce baud rate.

Schneider Electric M221 Pump Program
First you should check VFD input that can control by external switch or not. Then check sensor output that in type 4-20 mA and it connect to PLC analog card. I think this system PID are control by VFD not PLC, that why you don't see any PID configuration in PLC.
