My actuator is not moving and showing the attached faults.
all lamps are on.

 

Looks like a Torque fault on both open and close. Usually this happens when both limits were reached. What is the setting of open and close? Is it on torque? How much? For now, you have to loosen the gland packing and try to move to the close position (your indicator says it is fully open) by using the handwheel (your system should be isolated). Then press reset button if you feel it is loose. You also need to check your torque limits. If nothing happens, then you have to open your valve and check why it is seized.
